The implementation of standardized cytopathology reporting systems has significantly improved diagnostic consistency, risk stratification, and communication with clinical teams. However, indeterminate (“gray-zone”) diagnostic categories remain a major source of uncertainty and represent some of the most clinically consequential areas in daily cytopathology practice. This interactive, case-based workshop will focus on these challenging categories across six major reporting systems: pancreatic (WHO framework), salivary gland (Milan System), thyroid and gynecologic (Bethesda Systems), serous fluids (International System), and urinary cytology (Paris System). Rather than providing a comprehensive review of each system, the session will emphasize nuanced interpretation of borderline cases. Expert faculty will deliver focused presentations highlighting diagnostic criteria, common pitfalls, risk-of-malignancy data, and practical reporting strategies, with particular attention to downstream clinical management and ancillary testing. A highly interactive case-based session will reinforce key concepts through real-world examples and audience participation. This workshop addresses a critical gap in cytopathology education and provides practical tools directly applicable to routine practice.
